v Help loved one dress, bathe, provide steadying hand getting out of bed, off couch,    onto toilet.

v Make meals

v Change bed linens as needed

v Fill prescriptions, organize and give medications daily

v Connect IV medication and flush PICC line

v Check blood sugars and give insulin

v Connect night-time tube feedings and flush line

v Monitor transplant incision for signs of infection, possible wound care

v Take and record daily vitals

v Monitor your loved one for adverse reactions to medications

v Transport to Pulmonary Rehab and Clinic

v Keep in communication with the transplant team

v Make medication changes as directed by transplant team
